  • passport;
  • ID of the parent or guardian;
  • documents proving the right of an official representative (birth certificate or act from the guardianship authorities establishing guardianship);
  • receipt of payment of state duty;
  • two photographs 35x45 mm (matte, color).

A set of documents along with the application is personally submitted by a legal representative (parent, official guardian, trustee). The presence of the child is necessary.
