What is required to obtain a passport and how to obtain the application form

The official website of the Russian Ministry of Internal Affairs states that a Russian has the right to apply for one of two types of foreign passport:

  • old format, which is valid for 5 years;
  • a new sample with an electronic storage medium, valid for 10 years.

To become the holder of the selected type of international passport, the applicant collects and submits a package of documents:

  • to one of the departments of the Main Directorate for Migration Issues (GUVM MIA);
  • to a multifunctional center (MFC) of any region of Russia;
  • through the online service – Unified portal of public services.

Among the documentation that is submitted to the GUVM, the questionnaire deserves special attention, the correct completion of which will speed up the procedure for obtaining a foreign passport.

Documents for a foreign passport

  • Application form for an old-style international passport. You are allowed to fill it out on a computer or by hand. The adult applicant enters the data in accordance with the information specified in the children’s general civil documents.
  • Parents/representatives present their birth certificate as identification for a child under 14 years of age. The document must indicate the Russian citizenship of the small citizen. If there is no such mark, you need to take an additional certificate or affix the appropriate stamp.
  • Russian passport of the parent. If the last names of the minor and the applicant do not match, take a divorce certificate or other supporting document.
  • 2 photo cards 35 mm wide and 45 mm long. Printing on matte paper, can be in an oval, color or black and white. Face in profile, without headdress and toys. Eyes are open; glasses, if any, should not hide them.
  • Receipt for payment of the fee. The receipt is given by the passport officer or operator who accepts documents for the foreigner. It must be paid at Sberbank and added to the list of documents. Within three months after payment, you are allowed to change the information in your foreign passport – last name, first name – free of charge, if necessary.

General requirements for filling out the application form for a foreign passport

The contents of the application form for obtaining a foreign identity card are established by the Administrative Regulations approved by Order of the Ministry of Internal Affairs of Russia dated November 16, 2017 No. 864. The questionnaire form is contained in Appendix No. 1.

The application form for obtaining a biometric foreign passport and an old-style identity card are the same. And since the application form for issuing both types of passports is the same, the applicant simply marks with a cross in the application form which passport he needs.

The application form for obtaining a foreign identity card of any format is filled out according to the following rules:

  • Answers to questions are written by hand in block letters in black or blue ink. Errors and crossed out words are not allowed;
  • You can fill out the application on your computer using a text editor. To do this, you need to download the application form from the official resource, fill it out electronically and print it out;
  • in order to select the desired item in the answer to one of the questions, the applicant should put an “X” - a cross (“V” or other marks placed in the selected box will be considered an error) in the corresponding square;
  • in the questionnaire, the applicant indicates that he has no obligations (unpaid fines, alimony, credit, or other) that prevent him from leaving the Russian Federation;
  • The application form is filled out on one sheet of paper on both sides (and the reverse side). If you print out the application form on two separate sheets, the GUVM office will not accept it;
  • There is no need to paste the applicant’s photo into the field provided for this purpose - the photo is provided in a package along with other documents.

To avoid comments from an employee of the Main Department of Migration Department of the Ministry of Internal Affairs of Russia and filling out the form again, you can enter data into the questionnaire on the computer.

Due to the fact that official data is entered into the application for a foreign passport, in order to avoid mistakes, you must have the following documents with you:

  • national Russian passport;
  • existing valid or canceled passport (if any);
  • employment history;
  • confirmation of the change of first name, patronymic or last name (marriage registration certificate, certificate, court decision or other);
  • child's birth certificate (if you want to include children under 14 years of age in the passport).

The correctness and veracity of the information specified in the questionnaire minimizes the likelihood of refusal to issue a passport.

If you decide to submit an application form in electronic format using the Unified Portal, you must attach a personal photograph in electronic form to the correctly completed form.

Step-by-step instructions for filling out the application form for an old-style international passport for adults (over 18 years old)

A Russian citizen who has turned 18 years of age has the right to contact the authorized government body with a request to obtain a foreign passport in his name.

To do this, an adult Russian fills out an application form.

The questionnaire asks the applicant to answer 16 questions:

  • in the first paragraph you must indicate your full name in the nominative (not genitive) case;
  • the fifth question should contain information about the previous last name, first name or patronymic (if the applicant previously had a different full name);
  • in paragraph 6 “Residence address” the address of the registered place of residence (registration) indicated in the national passport is indicated;
  • Points 8 and 9 (telephone and email address) can be filled in as desired;
  • question 13 of the questionnaire is completed if the applicant already has a valid international passport of an old or new type;
  • in paragraph 14 on the reverse side of the application form, information about places of work over the last 10 years, including studies in educational institutions and military service, is entered. Information about the place of work (name of the company or organization) is indicated without abbreviations;
  • paragraphs 15 and 16 are noted by applicants who wish to include information about a child in their passport or submit an application in relation to an incapacitated person.

If the application form for obtaining an international passport of the old format is filled out by a pensioner, then the names and addresses of all enterprises (places of work) for the last 10 years are entered in section 14 of the application “Information on work activity over the last 10 years”. If a person has pensioner status for more than 10 years, then the table indicates the period of time during which the pensioner did not work (for example, 12.2014 - N.V. (present)), and the pensioner’s residential address is entered in the “Location of the organization” column.

Features of filling out an application form for a child’s international passport

The legislation of the Russian Federation provides for a separate application form for the issuance of a foreign passport to a minor citizen (under 18 years of age).

If a young Russian has reached the age of 14 to 18 years, he has the right to personally fill out and sign the application form, and then pick up the completed identity card at the appropriate branch of the Main Directorate for Migration.

If the child is under 14 years of age, the application for a foreign passport must be filled out and signed by one of the parents. In this case, information from the child’s birth certificate is entered into the form.

The application form for the issuance of a foreign identity card for a minor has two sections: information about the child and about the parent (legal representative). In paragraphs 1-10, the parent enters information about the child, and on the back of the document in paragraphs 11-24, information about the parent is indicated.

Instructions for filling out the old-style application form

1. Enter your personal data - last name, first name and patronymic.

2. Gender, put an X in the square, male or female.

3. Indicate the date, month and year of birth. Example "09/21/1972".

4. In this line, enter your place of birth exactly as it is written in your passport. Example "Mr. Samara".

5. If you did not change your last name, then enter “Did not change your full name.” If the surname was changed, then “Until 2000 Petrova, Moscow, Khamovnichesky registry office.” If the name has been changed more than once, all the information will not fit. To do this, today there is an application for changing personal data that must be filled out.

6. Write your permanent residence address and registration date in the appropriate lines, regardless of your location. If your passport does not have a registration stamp, leave all lines blank.

7. If you have a residence permit in another region, but are applying for a foreign passport in Moscow, fill out this item in accordance with the requirements. You have temporary registration in Moscow - put an “X” in the square of your place of stay and indicate the registration period. No registration - “X” in the square of the actual location in Moscow.

8. Indicate a telephone number where you can be contacted.

9. Provide an email address (optional).

10. Provide passport details. Please pay attention to who issued your internal passport. Write exactly the same in the application form.

IMPORTANT! Please carefully fill out paragraphs 11 and 12. If the information is inaccurate, you may be refused a passport.

11. If you have ever issued an international passport, please indicate the series, number, date of issue of the previous international passport, as well as the organization to which it was issued.

12. Fill out information about your studies and work activities for the last 10 years. Dates are written in the format MM.YYYY (for example, 12.2001); in the “Location” column, be sure to indicate the country, region, district, city (village, town, village, etc.).

IMPORTANT! If you have not worked or studied for more than a month, you must indicate this. If you are currently a non-working pensioner, then this must also be reflected in the questionnaire - “DID NOT WORK”. In the third column, indicate the place of permanent registration during this period of time. If, when filling out point 14, there are not enough lines to list all places of work, then fill out Appendix No. 2a to the application form for an old-style international passport.

13. If you want to enter information about children, you must fill out this item in accordance with Appendix No. 2b.

Types of attachments to an application for a foreign passport

The administrative regulations, approved by order of the Ministry of Internal Affairs of Russia dated November 16, 2017 No. 864, provide for four types of applications that supplement the application form for obtaining a foreign identity card.

If the application form does not have enough space to indicate all places of work, all changes in the applicant’s full name, or the applicant for an old-style foreign passport wants to include children in this document, the corresponding application is filled out.

Below are the forms for each of the four applications and examples of how to fill them out.

Appendix No. 2 “Information about changes in personal data”

This application is completed if the applicant changed his last name and/or first name.

Appendix No. 2a “Information on labor activity over the last 10 years”

Appendix No. 2a is completed if the applicant has changed many jobs over the past 10 years.

Appendix No. 2b “Information about children - citizens of the Russian Federation (up to 14 years old) for inclusion in the passport

Appendix No. 2b should be completed if you wish to include children in your foreign passport. It is worth noting that this is only possible if you issue an old-style international passport. The cost of the service is 500 rubles. Data about children is not included in the biometric passport.

The more convenient the old form of passport

An old-style international passport for a child under 14 years of age is a standard booklet of 36 pages, which contains basic information about the minor citizen. It is issued for 5 years, which, in view of the rapid changes in the child’s appearance, is rather an advantage.

The state fee for a foreign passport is 1,000 rubles, which is two times cheaper than the cost of a biometric document. In addition, to submit an application for an old-style international passport for a child, the presence of a minor is not required; parents or a representative can do this for him.

If the child is under 3 years old, then passport officers recommend making him an old-style international passport. The registration procedure is much simpler, the price is lower, and the validity of the document will expire before the baby has time to change beyond recognition. Parents will be able to avoid the hassle of replacing the ID (since the photograph cannot be changed individually) and proving the child’s identity at border control.
