An arrival address sheet is a document that confirms the fact of a person’s arrival in another city and is one of the list of documents required for temporary registration with the regional Office of the Federal Migration Service. It contains information about where the person came from and for what period, and also indicates the future place of residence of the foreign citizen. In Russian cities, two copies of the document are submitted. There is no state duty on these forms.
Important!!! According to the rules, these sheets should be filled out by civil servants who are responsible for registration, and not by citizens, but in practice, it is the people who are registering who fill out this document.
Why do you need an arrival sheet for a foreign citizen?
In order to make the process of obtaining temporary registration easier and to help citizens of the Russian Federation and foreigners, if they have a certain period of stay at their place of residence, to fully exercise their rights, on the basis of FMS Order No. 288 dated September 11, 2012 (as amended on January 19, 2015) a list of documents was created. It includes Form 2, an arrival address sheet, a sample of which is given in our material.
There is no state duty for this document. It should be submitted to the Ministry of Internal Affairs in accordance with the approved rules for registration and deregistration of citizens of the Russian Federation within the country (clause 17 of the Decree of the Government of the Russian Federation dated July 17, 1995 No. 713). Within three days, a document drawn up in accordance with all the rules will be issued by specialists responsible for this process.
Why do you need an arrival slip and where to get it?
The arrival address sheet (form 2) is filled out by citizens of the Russian Federation and immigrants before registering at a new place of residence or stay. After crossing the border, foreigners who plan to stay in Russia for more than 90 days are sent to the territorial office of the Ministry of Internal Affairs, where they fill out the specified form.
This document is also issued when changing place of residence (permanent or temporary) by Russians and immigrants who have already completed the registration . Since filling out Form No. 2 is a mandatory part of the registration procedure at the place of residence, parents of newborn children also resort to the procedure of signing the arrival sheet.
The address sheet contains information about where the applicant came from, where he moved and for how long. If a person applies for temporary rather than permanent registration, he must indicate the period during which he will reside at the new address.
Reference. The form of the arrival address sheet can be obtained from the Department of Migration Affairs (GUVM, former FMS) or downloaded on the Internet. It is issued free of charge, and no paperwork fees are charged.
How to fill out an address sheet
The document is filled out in duplicate by authorized FMS employees, but most often this is done by citizens themselves before submitting all documents. There is nothing illegal about this and is not considered a violation.
Considering this fact, you can fill out this document yourself as follows:

Before registering for a specific place of residence or temporary stay, first fill out the arrival address sheet. In this situation, the Directorate for Migration Issues of the Ministry of Internal Affairs of the Russian Federation (hereinafter referred to as the Directorate of the Ministry of Internal Affairs of the Russian Federation) is notified about entry to a new place of residence and about departure from the previous one.
When immigrating to Russia, foreigners and stateless persons also notify of their presence on the territory of the Russian Federation and fill out a similar form.
What is an arrival address sheet
The arrival address sheet (hereinafter referred to as the Address sheet) is considered to be the official certificate “Form No. 2”, which confirms the fact of a citizen’s move to another city or to a new place of residence.
When receiving a temporary residence permit, such a document must be submitted to the regional Department of the Ministry of Internal Affairs of the Russian Federation.
This certificate contains information about the previous city, the new city and the period of stay at a specific new place of residence.
A statistical arrival sheet is often confused with such a certificate. The main difference between them is that the latter is issued upon receipt of temporary registration at a specific place of stay for a period of more than 9 months.
What is it needed for
An address sheet is drawn up before registration at a new specific place of residence or temporary stay. When staying in Russia for more than 3 months, foreigners also provide such a document to the Department of the Ministry of Internal Affairs of the Russian Federation.
When changing your previous registration to a new one, Form No. 2 is filled out even after registration with the Department of the Ministry of Internal Affairs of the Russian Federation. Parents of one or more newborn children also draw up such a document.
Such a certificate includes specific information about the previous and future place of residence and the period of stay in the new city. When officially registering a new temporary registration (registration), this document must reflect the period of residence at a specific new address.
Form No. 2 is filled out and received at the local Department of the Ministry of Internal Affairs of the Russian Federation or downloaded from the Internet. There is no state fee for issuing such forms.
Who fills it out
According to the law, Form No. 2 must be completed by employees from the migration department of the Ministry of Internal Affairs of the Russian Federation. However, in practice, such certificates are filled out by citizens of the Russian Federation or immigrants themselves.

The address sheet is a double-sided document of the following dimensions: 105×145 mm. When downloading such a certificate from the Internet and completing it at home, these parameters are saved. The official form can be downloaded and printed in pdf and doc format.
A similar document is drawn up in 2 or 3 copies:
- 1st - for an individual applicant;
- 2nd - for the local (territorial) body of Rosstat;
- 3rd - for the Rosstat Office for a specific region of the Russian Federation.

Filling procedure
The address sheet is filled out independently as follows:
- in the first 3 paragraphs indicate the applicant’s full name only in the nominative case;
- in column 4 enter the date of birth (for example, July 7, 1991);
- in paragraph 5 indicate the gender of the person (male or female);
- in column 6, enter your place of birth in the following sequence: country, region of the Russian Federation, city (for example, like this: Russian Federation, Belgorod region, Belgorod);
- in subsection 7 indicate the presence of Russian citizenship;
- in column 8 enter the date and place of registration;
- in paragraph 9 indicate the government agency from the passport stamp with the registration address;
- In column 10, enter passport data (number, series, at what time it was issued and by whom it was issued);
- in subsection 11, the address of the previous place of residence must be indicated, indicating a specific region of the Russian Federation, district, city or other populated area, a specific street, apartment or private building, separate building (if any), apartment (for example, like this: Chelyabinsk region, the city of Magnitogorsk, Karl Marx street, 25, building 2, apartment 7).
On the second side of Form No. 2, fill out the following items:
- Column 12 is filled in when receiving temporary registration in the previous city, but at a different new address;
- in paragraph 13, data is entered on the change in the previous surname, name or patronymic after marriage or under other similar circumstances;
- additional (important) information is entered in column 14;
- in subsection 15 they put the date of execution of the document and a personal signature (for example, May 7, 2021).
The remaining points are completed by authorized employees of the Department of the Ministry of Internal Affairs of the Russian Federation.
Features of filling out a document for newborns
When registering a newborn child, Form No. 2 is filled out in the same way as a regular one, with the exception of key points:
- Column 8 - information about the future place of residence of the parent with whom one or more newborn children will live is entered here;
- 10 point - this subsection reflects various information from the official Birth Certificate of the child;
- Column 11 - no information is indicated here.
What to do if you lose
If Form No. 2 is lost, they go to the Department of the Ministry of Internal Affairs of the Russian Federation where a similar certificate was previously issued. This is done if, during state registration at a specific place of temporary stay, a citizen of the Russian Federation is required to provide a lost similar certificate.

The arrival sheet is a double-sided document measuring 10.5x14.5 cm. If the applicant decides to download the arrival address sheet (form No. 2) via the Internet and fill it out at home, then the specified parameters must be saved.

- the first is for the applicant;
- the second – for the territorial body of Rosstat;
- the third – for the main branch of Rosstat for the constituent entity of the Russian Federation. If the appellant resides in a subject center, he or she completes only two copies rather than three.
How to fill it out correctly
According to the established rules, the arrival sheet is filled out by an employee of the Ministry of Internal Affairs, but in reality this can also be done by the applicant. The form is filled out either by hand or on the computer at home, maintaining the original dimensions and parameters of the document.

Data indicated in form No. 2:
- Paragraph 1-3: Full name of the applicant.
- Item 4: Date of birth.
- Point 5: Gender.
- Item 6: Place of birth.
- Clause 7: Citizenship.
- Clause 8: Information about permanent registration (registration) in Russia or abroad. If the arrival sheet is filled out for a newborn, indicate the address of one of the parents with whom the child will live.
- Clause 9: Registration body registration - the name of the department in which the registration was registered.
- Clause 10: The name of the identity document and its details (for example, a civil passport or a minor’s birth certificate).
- Point 11: Place of departure, i.e. where the migrant came from - country, city, address. If the applicant is the parent of a newborn, he does not enter anything.
- Clause 12: Temporary registration address in the same city where the applicant is registered - if the applicant is registered in another city, the line is left blank.
- Item 13: Line No. 13 is filled out if the migrant has previously changed the data from items 1-5.
- Paragraph 14: Indicate important information, for example, the reason for changing personal data.
- Item 15: Date of document preparation and signature of the applicant.
- Item 16: Date of registration of the appeal - filled out by an employee of the Ministry of Internal Affairs.
The information indicated on the arrival sheet must completely coincide with the data in the migrant’s documents: passport, migration card, departure sheet, etc.
A migration service employee will check the form for correctness and accept it for processing, which takes 3 days. The main mistakes made when filling out the form:
- illegible handwriting;
- grammatical and factual errors - discrepancy between data and information in documents;
- the form is not printed correctly;
- presence of strikethroughs and corrections;
- The form is filled out in a single copy.
What to do if you lost
The arrival certificate serves as confirmation of the arrival of a Russian or foreigner at a new place of residence, but is not always indicated in the list of necessary documents for obtaining a new residence permit.
If, when registering at the place of residence, an immigrant is required to fill out Form No. 2, which he lost, the foreigner contacts the department of the Ministry of Internal Affairs, where he previously filled out the arrival form.
