When changing his place of residence, a Russian can receive a certificate of departure at his previous address of residence. If such a document is lost, the person notifies in writing of the circumstances due to which the document cannot be presented. This is done at the newly chosen place of residence.

Important! However, the absence of an act of departure does not become an obstacle to the submission of application documents for the chosen place of residence and in subsequent procedures.

People become removed from the account:

  1. In a place of short-term stay in housing not intended for their long-term residence - at the end of the period of such stay.
  2. In hotels, hospitals, tourist centers and other accommodation organizations for guests - upon their departure.

There is no need to apply for de-registration at the place of short-term stay from an individual and to draw up an act of departure using form No. 7. An exception is if persons registered at the place of short-term stay intend to leave there before the end of the time specified by them earlier.

When discharging from medical institutions persons registered in them at the place of short-term stay, an authorized employee of this institution, within 3 days from the date of discharge, draws up departure certificates for them.

Number of copies - 2:

  • The 1st document is sent to the local branch of the address bureau;
  • 2nd - transferred to the regulatory authority.

If the FMS refuses to establish the status of forced migrants on the basis of messages from the FMS, at the end of 3 months from the date of registration of such persons at the place of stay in places of temporary settlement, the regulatory authorities remove them from the registration system in the manner established by the Administrative Regulations.

A mark on the removal of citizens from registration at the place of residence is placed on 2 copies of departure slips of form No. 7 (Appendix 7 to the Regulations), which are issued to citizens along with an identity card.

The document is a 15-item questionnaire.

Points 1–11 are located on the front side. Information must be entered into them. All items are extremely simple and do not raise any questions when filling out.

The reverse side contains items 12–17. The twelfth paragraph is required to be completed here. The rest are filled in as needed.

The employee responsible for the correct completion of the document puts the date and signature on it. This means that all the information specified in the document is verified and reliable, and the sheet itself is formatted correctly. If you deregister from your previous residential address, a corresponding note will be added to your passport.

In what cases is it necessary

Registration of a departure address sheet in Form No. 7 is carried out if a person temporarily registered at a certain address intends to leave it before the deadline established in accordance with the certificate of temporary registration.

If a person is discharged from any medical institution, the authorized person of this organization, within three days from the date of discharge, must draw up a departure address sheet in form No. 7 in two copies, after which one of them is sent to the appropriate address and reference unit of the nearest territorial body, while the second one will be sent to the registration authority.

If the migration service or its individual territorial bodies refuse to grant citizens the status of forced migrants, then after three months from the date of registration of these citizens at their place of residence, they are deregistered, and the corresponding stamp is placed on the second copy of the departure slip .

How to deregister

Current legislation specifies as a place of residence certain real estate in which a citizen lives on a permanent or temporary basis, and if a person plans to move, he will need to carry out the re-registration procedure.

To be deregistered, he needs to contact the territorial migration service office, provide a complete package of documents along with his passport, and then receive it back with the appropriate stamp.

It is worth noting the fact that this procedure can be carried out in two ways - an extract at the previous address and registration at a new one, or carrying out all procedures at once in a new branch of the migration service.

It is quite natural that the second option is more optimal, since it allows you to significantly reduce the time required to visit the registration authority, and all procedures related to deregistration will be carried out independently by employees of this service without the need to involve the registered person.

Changing the address of residence is associated with the procedure of deregistration at the previous address of residence and registration at the new place of residence. One of the documents often requested when completing such a procedure is a departure slip. The article discusses the mandatory availability of such a document under the legislation of the Russian Federation and provides a sample of filling out the departure address sheet (Form 7).

To deregister at your place of residence, you must personally contact the person responsible for receiving and transmitting documents to the regulatory authorities (this can be an MFC official) and provide the following documents:

  • passport;
  • application for removal from registration in any form;
  • departure address sheet.

Why do you need a departure slip and where to get it?

The departure address sheet (form 7) is included in the list of documents developed by the Main Directorate for Migration of the Ministry of Internal Affairs of Russia (formerly the Federal Migration Service) for the registration procedure at a person’s place of residence/stay in the Russian Federation.

Registration is carried out by the Main Directorate for Migration of the Ministry of Internal Affairs of Russia. To obtain an extract, you must contact the local migration office or any of the government bodies that provide intermediary services between the citizen and the migration service:

  • passport office at the housing office;
  • MFC;
  • State Services portal.

Important! Only during the self-discharge procedure does a person receive a departure address sheet.

When a minor is discharged, all documents are drawn up for him by his parents, adoptive parents or legal guardians, subject to permission from the guardianship authorities.

At the new place of residence, a person submits his passport for registration along with the departure address sheet.

Procedure

You can de-register immediately before leaving for a new place of residence; This can also be done while simultaneously submitting documents for registration in a new place. The standard form of the departure slip is two-sided, and the citizen must fill out this document on both sides.
The form of this document is approved by current legislation and initially it is already prepared for filling out. The citizen only needs to indicate the required information in the appropriate fields. Initially, the full name and date of birth of the person being discharged are indicated in the corresponding lines, after which the information in accordance with the passport data is indicated in the next field “citizenship”. In this case, you need to take into account the fact that each country has its own national passport.

Without fail, the departure slip must contain detailed information about the place of birth of the person being discharged, and initially on the form you can even see a certain list of options, from which the most optimal ones are selected. Thus, if a person was born in a city, he selects the city in the appropriate field, and if in an aul, he selects an aul, and the full geographical name of the specified locality, as well as the gender of the person being written out, must be indicated.

After this, you can begin to describe the housing in which the person was registered. In this case, you will also need to indicate the full name of the locality and the full address of the housing itself, including the house number, building number and all other details.

Next, it is mandatory to fill in information about where exactly the person has moved, and in this case it is necessary to describe the locality in as much detail as possible, indicating not only its name, but also its type (village, town, city, etc.).

If a person carries out the procedure for deregistration and registration while in a new locality, he can make the appropriate mark on the departure sheet. The same applies to situations related to changes in personal data.

Now you can turn the sheet over and fill out the back side, which indicates information about your identity document (in most cases, this is a civil passport).

If minor children are moving with a citizen, you will also need to indicate all the relevant information about them, but the data should no longer be as detailed as in the case of the citizen being discharged. Basically, migration service employees are only interested in the child’s full name and date of birth.

As a result, you will need to put in the appropriate certificate and submit the papers for verification to an authorized employee of the migration service department. This person must necessarily endorse the document with his own signature, and then independently fill out all the fields relating to his department, after which the citizen can be considered officially registered at the new address.

How to fill it out correctly

When deregistering on your own, you must write an application (sample) and fill out Form 7 in two identical copies. How to correctly fill out the Form 7 departure address sheet?

Important! The application and Form 7 are filled out only on the basis of documents (general passport, birth certificate for minors, etc.) of the person who is being discharged from this living space.

There are 17 points on the departure sheet, of which 1 to 12 are required, which reflect the following information without abbreviations or errors:

  1. Name.
  2. Surname.
  3. Middle name (if available).
  4. Date of birth in the format “DD, MM, YYYY”: the month is written in words, the day and year – in numbers.
  5. Floor.
  6. Place of Birth.

  • indication of data on changes in full name and other personal information;
  • the reasons why personal data was changed;
  • date of the document and signature of the applicant;
  • date and signature of the official.

What to do if you have lost your departure slip?

To register at a new place of residence, a package of necessary documents is provided. The departure address sheet is not considered mandatory in this package. Its presence is desirable, but not required.

Important! If a person does not have Form 7 (for any reason, including loss), the employee does not have the right to refuse to accept documents for registration at a new place of residence.

A citizen applying for registration writes a statement about the reasons for the absence of Form 7.

If you still need the lost departure address sheet (Form 7) for other purposes, you can restore it at the place of discharge - write an application and receive a duplicate.

The moving process is associated not only with the physical movement to a new place of residence, but also with the documentation of this fact, which is defined by law as a person’s obligation to the state.

When you register on your own, you receive your passport with a discharge stamp and a departure address sheet. If the extract is carried out when registering at a new place of residence, Form 7 is actually not needed.

Citizens may be considered deregistered at the place of stay in housing that is not their main place of residence, in the event of the end of their stay, as well as after leaving sanatoriums, hotels and other similar institutions.

An application for deregistration, as well as obtaining a departure address sheet, is not required in this case, except in cases where the citizen intends to leave the place of temporary stay before the end of the established period.

When discharged from medical institutions, registered citizens must receive a departure address sheet based on Form No. 7.
