Application for a passport replacement in 2021: filling out the form

Passport replacement is one of the standard procedures of the passport and visa department of the Ministry of Internal Affairs. In addition to the scheduled issuance and change of document, replacement is usually required when the passport is damaged or lost. The application should be submitted using the standard Form 1P. The issuance period is up to 14 days.

Which authorities should I contact?

The application is processed by the GUVM department (formerly the FMS). There, the passport is issued in person and documents are verified. Any other methods are only variants of the main one using intermediaries.

Important! Lack of registration is not a basis for refusing to accept an application.

You need to go directly to the department of the Ministry of Internal Affairs if the document has been lost. In this case, it will be immediately entered into the database and, if necessary, a temporary identity card will be issued.

In less problematic cases, the request can be sent through the MFC or the State Services website. The advantages of the centers are that there are no queues and no connection to the place of registration; the state portal allows you to apply for a replacement directly from home, leaving one visit to the passport office to verify documents and issue a new passport.

Where to apply for a replacement

Form 1P can be obtained from the GUVM office, MFC or filled out online. In addition to the application form itself, you will need an old passport (if you have one) and two standard photographs. The forms are free and issued by institutional employees.

If the applicant is unable to visit the GUVM department due to health problems, he must submit a request for a visit to the passport office in a written form. After examination, a migration service employee will visit the patient and help him submit documents at home.

The appearance of the form was approved by order of the Ministry of Internal Affairs No. 851 in the first appendix. The form itself is a medium-sized card, 21 by 15 centimeters, filled out on the front and back sides.

At the end, the application form is signed by the applicant. When sending a request through State Services, the signature is the fact of logging in with your details.

There are four main reasons for applying to the GUVM for a new passport:

  • The document is unsuitable for further intended use;
  • Change of surname or first name;
  • Radical changes in appearance;
  • Reaching 14/20/45 years.

Wear and mechanical damage

A passport is considered damaged if key entries, marks or stamps in it have become unreadable. A document with lost pages or traces of exposure to fire or water must be replaced. Any notes or recordings not provided for by law are also prohibited.

Important! Russian legislation in fact does not distinguish between the loss and wear of a document, so sometimes it is easier to file a claim for the loss of a passport than to bring paper that has been worn out in the washing machine for a replacement.

Change of full name

A citizen of Russia has the right to change his name, surname and patronymic upon reaching the age of majority. Despite the opportunity, it is rarely used directly. The vast majority of replacements are associated with a change of surname upon marriage, in rare cases - upon adoption of a child who has reached the age of 14.

Requirements for filling out an application

The document is filled out by hand or printed on a computer by the applicant. The text should be written using black ink and be clear and neat. Corrections, non-standard abbreviations and erasures may result in refusal of admission.

In practice, several points cause the greatest difficulties:

  • Marital status is indicated as accurately as possible. “Not married” is used only for those who have never registered their marriage or have not been married;
  • The current registration is entered in the “Place of Residence” column, and current housing is entered in the “Place of Stay” column;
  • “Provided document” indicates the details of the document provided to the passport office as the basis for issue/replacement.

It is allowed to enter additional information in the empty fields by hand, even if the rest of the document has been printed.

Step-by-step instructions for filling out the application

The application must indicate:

  1. Code of the department for which the questionnaire is intended;
  2. Filling date in the format “dd.mm.yyyy.”

  3. Personal data - full name, gender and date of birth of the applicant, marital status;
  4. Full names of both parents of the applicant;
  5. Place of residence according to permanent registration (registration);
  6. Date of receipt of Russian citizenship (if you were previously a citizen of another country);
  7. Reason for replacing or issuing the document.

Important! Despite its presence, the “passport data” line does not need to be filled in in most cases. It is assumed that the document is lost or rendered unreadable, so the citizen simply has nowhere to get this information.

What to do if you make a mistake

If there is an error in the application, there is no need to try to forward or cross out the incorrect lines; this will only lead to wasted time and a decrease in the chances of the application being accepted.

If a mistake has already been made, you need to write the application again. If you submit an application with an error, you must contact the passport office again to correct it.

The worst situation that can occur in the event of an erroneous application is that the citizen will still be issued a passport, but with the wrong last name or the child’s first name.

In this case, it is best to immediately re-apply for a replacement, getting away with losing a couple of weeks and an additional fee.

The exchange and issuance of passports is one of the most common procedures performed by passport office employees. Most problems that arise are resolved by GUVM specialists on the spot, without additional delays.

Section 9 indicates the reason for obtaining the passport. Application forms can be downloaded free of charge from us, obtained directly from the migration department or from employees of multifunctional centers. Form size: 210 x 148 mm, according to the Regulations it must be filled out on punched card paper.

However, in practice, migration departments use ordinary thick paper of white or light gray color.

When you print out the completed form, we recommend using thick white paper (cardboard); regular photocopier paper will not work in this case.

This is due to the fact that forms No. 1-P are stored for a long period (75 years) and thick paper will ensure their safety during this period. The application must be filled out carefully by hand or typewritten (i.e.

filled out electronically and printed on a printer) by a citizen who personally applied for a passport.

Entries in the application are made in legible handwriting (block letters) using purple or black ink (paste).

Corrections in the application are not allowed. Clause 11 indicates the name of the document, its series, number, when and by whom it was issued, which served as the basis for the issuance (replacement of the passport).

The applicant’s personal signature is certified by the employee accepting the documents, so there is no need to sign the application in advance. It is better to do this in paragraph 10 of the application in the presence of an employee.

If for some reason a person cannot fill out the application on his own, then it is filled out by the employee accepting the documents.

If you have a passport issued by the Belarusian competent authorities before August 17, 2002, you must also submit a certificate from the territorial office of the Federal Migration Service of the Russian Federation at the place of registration, confirming the absence of Russian citizenship. If you have Russian citizenship, the certificate must indicate the exact date of its acquisition.

In accordance with the Regulations on Identity Documents, approved by Decree of the President of the Republic of Belarus of June 3, 2008 No. 294 “On documenting the population of the Republic of Belarus,” every citizen who has reached the age of fourteen is required to have a passport.

A citizen permanently residing outside the Republic of Belarus, as well as traveling outside the Republic of Belarus, is required to have a passport regardless of age.

A passport is issued to a citizen:

  • in connection with reaching the age of fourteen;
  • a minor under fourteen years of age, at the request of one of his legal representatives;
  • when acquiring citizenship of the Republic of Belarus;
  • in case of loss (theft) of a passport;
  • for permanent residence outside the Republic of Belarus.

The issuance and exchange of a passport is carried out: - to a citizen living in the Republic of Belarus, - by the internal affairs body at the citizen’s place of residence; - a citizen permanently residing outside the Republic of Belarus and registered at the consular office - by the Ministry of Foreign Affairs.

To issue a passport for permanent residence outside the Republic of Belarus, a citizen of the Republic of Belarus permanently residing in a foreign country must present the following documents:

Renewing a passport in Belarus

The validity period of the passport is limited. This is due to age-related changes in a person’s appearance, as well as the following reasons:

  • changing the premarital surname to the marital surname, as well as the name or patronymic;
  • Damage to a document due to falling into water, chemical destruction, increased wear, physical damage (torn pages, etc.);
  • detection of inaccuracies in records;
  • filling all pages with marks;

A Belarusian passport is valid for 10 years from the date of issue - for citizens under 64 years of age. After that, it is valid until the owner reaches 100 and 125 years of age. But for traveling abroad, a passport issued more than 10 years ago may be unsuitable and will have to be replaced.

A citizen has the right to exchange his passport prematurely if the validity period expires in less than a year.

Deadlines for passport replacement

In a standard situation, the passport is changed within a month. For an additional fee, the document can be produced in an expedited (15 days) and urgent (week) manner. This is the maximum acceptable time frame, but in practice the document is often produced faster.

How much does it cost to replace a passport?

Citizens of Belarus who are on state support can change their passport free of charge.

In other cases, the standard state duty is 1 basic unit. For urgent replacement, 3 basic amounts are paid.

  1. Statement. It is compiled at the passport office; you can take the form on site or download it below on the page and print it on thick paper. For a fee, an application can be filled out by an O&M employee.
  2. Old passport.
  3. Four color current photos 4x5 cm, made on one sheet (cutting is not allowed).
  4. Birth Certificate (will be returned).
  5. A document confirming payment of the state duty.

Married persons submit a document confirming its conclusion, as well as birth certificates of children (if any).

Divorced citizens must present a divorce document, and widows/widowers must present a death certificate of their spouse.
